- Organization Information
- Full name, abbreviated name, organization email, category
- Officer Information - You must submit all members who hold a position (up to 20)
- President, Treasurer, and Event Planner positions are required
- Name, position, net id, horizon email, phone
- On-Campus Advisor Information
- Name, CSUEB email, title, department
- Off-Campus Advisor information if applicable*
- Upload the Constitution and Bylaws ()
- Organization's constitution must be reviewed annually
- The constitution must include the following areas (similar or equivalent): Name, Purpose, Authority, Membership, Officer (duties), Selection Process of Officers, Meetings, Advisor, Banking/Finance, & Amendments
- If the constitution is missing any required areas, the organization will NOT be recognized until documents are approved by SLIC

Officers of the student organization must be elected/appointed as outlined in the organization's constitution/bylaws. The President, Treasurer, and Event Planner are required positions will be authorized by the organization to be page administrators to submit events/reservation requests and have access to the RSO’s AS Account, pending completion of Officer Training.
The President, Treasurer, and Event Planner MUST:
- Be a current enrolled student at least half-time (undergraduate 6 units / graduate 4 units).
- Have a minimum 2.0 GPA.
- Be in good standing with no academic alert or probation of any kind (academic, disciplinary, or administrative).
- Complete Officer Training Components: Nuts & Bolts Sessions, Canvas Modules, and Attend Fall Leadership Conference / Spring Leadership Symposium.

Each officially recognized student organization must have a university advisor who is either a faculty member or professional staff member who is an employee of the CSU or one of its auxiliaries. Campuses may permit part-time faculty and professional staff to serve as advisors ().
CSUEB faculty/staff, or a CSUEB auxiliary member can qualify to be an On-Campus RSO Advisor.
Who cannot serve as an On-Campus advisor? Undergraduate students who are student employees OR a graduate student who is also professional staff and is a current member or officer in the organization they are requesting to advise.

Student orgs can have Co-Advisors, both advisors must complete the [RSO] On-Campus RSO Advisor Agreement Form.
- Log into BaySync () with your Net ID and password.
- Navigate to “Forms” and locate the “BaySync” tab.
- Click on the form named [RSO] On-Campus RSO Advisor Agreement Form.
- Complete and submit the form for review by SLIC RSO Staff.
